1. Terms and conditions
I, the undersigned hereby declare and consent to the following:
1. Accuracy of Information
I declare that all information provided in this enrolment form is true and correct to the best of my knowledge. I understand that if any falsehoods or omissions are discovered, I may be de-enrolled from the programme and expelled from Harmony and Help Academy.
2. Participation Commitment
I undertake to fully participate in all structured learning, practical workplace experience, and assessment activities as required by the programme
3. Consent to Personal Information Processing
I consent to the collection, processing and storage of my information by Harmony and Help Academy in compliance with Protection of Personal Information Act, 2013 (POPIA)
4. POPIA Rights
I am aware of my rights under POPIA, including right to access, correct, or request deletion of my personal information
5. Purpose of Personal Information
I understand that my personal information will be used for assessing if I meet the requirements for entry to the programme, ability to comprehend and manage course content. I acknowledge that it will be treated as confidential and shared only with relevant third parties on my permission.
2. Terms and conditions
I. Student must pay the full deposit in order to be recognised as a registered student at the Academy.
II. In the event that the student wishes to cancel the contract, he/she shall provide within 7 days, a dated written notice from the date of registration thereof to the Head of Academy.
III. Refunds will only be effected according to the Institution’s Refund Policy and the student must make a formal application in writing providing a detailed explanation for the refund, to the Head of Academy.
IV. The onus lies with the student to retain a copy of the letter submitted to the Head of Academy as proof of refund request.
V. In addition to the Academy’s Refund Policy, the refund will only be payable within 15 working days from date of request.
VI. Should a student drop out, the deposit and any other instalments paid will be forfeited, in addition, the student will be liable for all remaining fees.
VII. Any over payments or refunds that is due to a student must be claimed by student within 6 months of the date of last payment.
VIII. Students who pay their deposit, complete the enrolment form but do not attend classes will be handed over to the debt-collectors.
IX. In the event that a student stops attending classes for any reason whatsoever, including expulsion, he/she will remain liable for the full fees and will not be entitled to any refunds.
